As a nursing student, I have gained a solid <br /> foundation in community health, and worked with social workers and case managers to connect <br /> patients with community based resources. I have planned and implemented evidenced based <br /> interventions for diverse communities. I have organized sustainable campaigns that empowered <br /> individuals to make informed decisions. Engaged in fireside chat conversations with community <br /> members to talk about homelessness, domestic violence, and exploitation of our aging <br /> community members. As a domestic violence survivor myself, I knew that the only way that I <br /> survived and thrived is because of the help and resources that I was connected to. I want to be <br /> able to contribute the same to our community members. <br /> I also volunteer as a postpartum doula to families and single parents that cana€TMt afford a <br /> doula service after given birth. Connect them to accessible community resources for diapers, <br /> food, baby clothing and many more. I have a good understanding of complex medical conditions that affects <br /> seniors in our community, this can directly contribute in ensuring that residents in nursing homes <br /> are receiving evidence-based care. Also, I have experience with advocating for seniors and/or <br /> adults with disability both in healthcare settings, and in the community, and also with <br /> communicating with patients, families, and multidisciplinary team. I have received dementia care <br /> training, and can effectively conduct assessments like functionality assessments (ADLs & <br /> IADLs), depression screening, cognitive screening, medication reconciliation using the BEERs <br /> criteria and many other geriatric assessment.
