Since this is a witness agreement, <br /> there are two (2) options to complete the agreement. It is acceptable to have everyone sign the agreement <br /> and submit the scanned copy electronically, or to complete electronic signature documents as provided by <br /> ADFP Trust Fund staff at the signatory's request. To return a hand-signed and scanned agreement packet, <br /> or to request a digital/electronic signature agreement packet, email ADFP Trust Fund Grant Specialist <br /> Kelsey Pearce at kelsey.pearce(&ncagr.gov. <br /> By completing these documents, you are agreeing to the specific stipulations, the general terms and <br /> conditions and specific reporting requirements. For hand signatures, all authorized representative <br /> signatures must be in blue or black ink. Please use the Agreement Check Off List to ensure all <br /> attachments are included and are in the correct order for each agreement packet. <br /> One fully-executed, original agreement will be returned to you for your records.
