EXHIBIT A: Scope of Services <br />IFC Permanent Supportive Housing <br />7/1/24-12/31/24 <br />$75,000 <br />Expense Description: <br />IFC will use Orange County funding to expand its PSH program by: <br />•Creating a Team Lead position (1 FTE) to maintain a caseload and assist members in <br />maintaining housing; provide consultation, training, and back-up support to case <br />managers; locate housing units; coordinate service delivery; and assure complete and <br />accurate record-keeping and data management. <br />•Providing rental assistance for an expanded number of units on the caseload. The number <br />of units expanded will depend upon the size of households needing services and the cost of <br />available units, especially given current Fair Market Rents (FMRs). <br />•Ensuring compliance with HMIS and HUD record-keeping requirements. <br />•Using funds as available for critical household needs not covered by HUD funding, such as <br />security deposits, moving and move-in costs, unit clean-up, damages, other household <br />goods when donations are not available, and arrangements when members pass away. <br />Docusign Envelope ID: 93E9F37E-6738-4E9C-B982-F46B7B277746
