Rev. 7/1/2024 Page 1 of 3 <br /> <br /> <br /> <br />CONTRACT AMENDMENT # 1 TO THE AGREEMENT <br />BETWEEN ORANGE COUNTY <br />AND ALLIANCE HEALTH <br /> <br />THIS CONTRACT AMENDMENT dated is October 23, 2024, between Alliance Health (hereinafter <br />referred to as “Alliance”), and the County of Orange, (hereinafter referred to as “County”), with regard to <br />the CARE Team Pilot Program. <br /> <br />WITNESSETH: <br /> <br />THAT WHEREAS, Alliance and County entered into a contract dated June 1, 2024, for the provision <br />of Crisis Assistance, Response and Engagement (CARE) Team, (hereinafter the “Original Agreement”); <br />and <br /> <br />WHEREAS, Alliance received an Allocation letter from the NC Department of Health and Human <br />Services in the amount of $579,500 for continuation of the services set forth in this agreement in FY 25; <br />and <br /> <br />WHEREAS, Alliance and County desire to amend the Original Agreement, while keeping in effect <br />all terms and conditions of the Original Agreement not inconsistent with the terms and conditions set forth <br />below. <br /> <br />NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, <br />the parties agree to amend the Original Agreement as follows: <br /> <br />1. Section 1. Funding, is amended to read as follows: <br /> <br />Alliance shall provide to the County funding in the amount not to exceed Two Hundred Twenty- <br />Nine Thousand Five Hundred Dollars ($229,500.00) for the startup funding expended between April <br />1, 2024 and June 30, 2024, and an amount not to exceed Five Hundred Seventy-Nine Thousand Five <br />Hundred Dollars ($579,500.00) for eligible expenditures during the period of July 1, 2024 through <br />June 30, 2025. (hereinafter referred to as the "Funds") to support staffing of the Team, training, and <br />additional equipment and supplies for the 2024-2025 fiscal year. A portion of this funding has been <br />allocated by DHHS and is subject to Allocation Letter 25-A-31, incorporated herein by reference <br />thereto. <br /> <br />2. Attachment 1, Scope of Work, Finance, is amended to read as follows: <br /> <br />FY 2024: Start-up costs of an amount not to exceed $142,000, and $87,500 in start-up operational <br />staffing costs for a total of $229,500 in non-UCR funding. <br /> <br />FY 2025: An amount not to exceed $579,500 annual operating costs, subject to the Department <br />Allocation Letter 25-A-31. <br /> <br /> <br /> <br />       <br />Docusign Envelope ID: 317F49C9-77A7-4EB0-8229-FB228B2131CC
