<br /> <br />252-430-1503 <br />866-640-2028 <br />Providers of Quality Built Emergency Apparatus <br />CONTRACT FOR ORANGE COUNTY EMS TYPE II OSAGE AMBULANCE <br />Date: 9-10-24 <br /> <br />Southeastern Specialty Vehicles, Inc., (Seller) hereby proposes and agrees to furnish to Buyer, after <br />Buyer’s acceptance of this proposal and the proper execution and approval of the accompanying work <br />order/contract, the following: <br /> <br />Osage Ambulance Type II Ford T350 HR AWD <br />Orange County EMS Custom Graphics <br />110V Heater in Attendant Seat Base <br /> <br />Purchase through the BuyBoard purchasing co-op <br />Buyboard Contract number 745-24 <br /> <br />Product and Services: <br /> <br />All of which are to be built in accordance with the attached specifications, and which are made part of <br />this agreement and contract, to deliver same approximately (24-30 Months) from receipt of the <br />following, subject to all causes beyond our control: <br />(1) This signed contract, <br />(2) Chassis delivered to manufacturer’s location, <br />(3) Your executed Purchase Order meeting these terms and specifications and, <br />(4) An approved Work Order with all specifications, properly executed, <br /> <br />Price: For the sum of $160,345.00.00 (_____) Initials. <br />One Hundred Sixty Thousand Three Hundred Forty-Five Dollars & Zero Cents <br /> <br />Terms: The Price for the Product is F.O.B.: Henderson, North Carolina <br /> <br />Payment Terms: Payment in full is required at time of final inspection to be held at Southeastern <br />Specialty Vehicles Inc., located in Henderson, North Carolina 27537, or at the manufacturer of record. <br />Late payments are subject to a $1000.00 Fee plus daily interest of 18% APR, unless prior arraignments <br /> have been made with President of Southeastern Specialty Vehicles, Inc.
