Games, <br /> puzzles were in short supply. Activity <br /> calendar was sparse. This facility could <br /> benefit with more community <br /> participation. Several residents <br /> mentioned they were bored. <br /> 16. Do residents have the opportunity to purchase <br /> personal items of their choice using their monthly YES <br /> needsfunds? <br /> Can residents access their monthly needs funds at <br /> their convenience? <br /> 17. Are residents asked their preferences about YES Administrator indicated there were <br /> meal/snack choices? choices, but many residents did not <br /> Are they given a choice about where they prefer to express themselves. <br /> dine? <br /> 18. Do residents have privacy in making and receiving YES <br /> phone calls? <br /> 19. Is there evidence of community involvement from NO The facility is currently in transition with <br /> other civic, volunteer or religious groups? a change in administration and activity <br /> director and front desk staff. <br /> 20. Does the facility have a Resident's Council? YES The President indicated things were in <br /> Family Council? No transition and seemed very patient. <br /> Areas of Concern Yes/No/ Exit Summary <br /> NA <br /> Are there resident issues or topics that need follow-up or <br /> review at a later time or during the next visit? Upcoming will be the 4t" administrator <br /> this facility has had this year. The <br /> interim administration staff will be <br /> leaving shortly. <br /> The activity program has had a set back <br /> with a transition as well. This program <br /> aspect is seriously lacking and residents <br /> expressed boredom. They at least need <br /> self-directed activity which does include <br /> the TV! <br /> The residents do not seem to have <br /> access to beauticians. This would help <br /> the residents if they had access to <br /> haircuts, etc. <br /> It was mentioned that a lot of residents <br /> were still in bed in pajamas with <br /> breakfast trays. We arrived at the <br /> facility at 10AM. <br /> One resident expressed interest in <br /> getting to her churches homecoming. <br /> We asked that this be shared with the <br /> social worker. Miss Kim indicated that <br /> transportation was at least $20.00 and <br /> she would see if the resident account <br /> allowed it. <br /> This Document is PUBLIC RECORD.
