The activity director also left <br /> along with the front desk receptionist. <br /> • The survey book was up to date, <br /> but needs to be displayed in a <br /> more visible area. <br /> • One resident was unhappy with <br /> the hardened bread she was <br /> served. The administrator <br /> indicated that staff could ensure <br /> softer toast, but the resident <br /> needed to express this to them. <br /> • The administrator indicated the <br /> residents have gotten use to <br /> being served their meals in their <br /> room because of the fire and <br /> construction that followed. She <br /> asked for methods to get the <br /> residents to the dining room and <br /> several suggestions were made. <br /> • Some of the residents were <br /> observed to be in bed still in <br /> their bedclothes while staff <br /> chatted in small groups. The <br /> administrator indicated she <br /> would address this with <br /> supervisors and staff. <br /> • The courtyard was addressed. <br /> The plantings were dead and the <br /> lawn furniture was broken and <br /> presented a safety risk. <br /> • Census is low. 140 bed facility <br /> provides care for 83 people <br /> currently. <br /> This Document is PUBLIC RECORD.
