z <br />c, Long Term Care Screening (EDS) -this State allocation of federal funds totaling <br />$10,000 will be used to enable low-income, minority individuals to defer or prevent <br />institutionalization by providing quality services to meet basic needs. These <br />additional funds bring the current budget to $35,000 for FY 2003-04, This budget <br />amendment provides for the receipt of these funds. (See Attachment 1, column #1) <br />d. IV-B Adoption Vendor Payments -these additional State funds totaling $5,000 are <br />payments for allowable costs on behalf of children who are eligible for adoption <br />assistance. These additional funds bring the current budget to $20,000 for FY 2003- <br />04. This budget amendment provides for the receipt of these funds. (See <br />Attachment 1, column #1) <br />Department on Aging <br />2, The Department on Aging has received additional revenues in FY 2003-04 far Adult Day <br />Care operations related to private pay revenues totaling $12,000, as well as reimbursement <br />funds from the Home and Community Care Block Grant program totaling $9,052. These <br />funds will be used for personnel costs related to the Adult Day Care operations. This budget <br />amendment provides far the receipt of these funds, (See Attachment 1, column #2) <br />Health Department <br />3, During the budget review process for fiscal year 2003-2004, the Board of County <br />Commissioners expressed a desire to continue several of the Health Department's grant <br />supported programs that lost funding or were experiencing reductions in funding. These <br />programs and their associated positions were extended with the balance of their revenue <br />shortfall to come from the Human Services Social Safety Net. The following are these grant <br />supported programs: <br />Enhanced Child Service Coordination <br />Provides a central portal of entry to services for children with developmental delays as well <br />as ensures that appropriate referrals to agencies throughout the County are made. Case <br />Management is also provided for the more complex cases. It receives Smart Start funding in <br />the amount of $46,725. <br />Community Health Awareness Project (CHAP) <br />Provides the only outreach program to the Hispanic community and other hard-to-reach <br />populations by training lay health advisors who provide information to neighbors and <br />acquaintances (36 lay health advisors have been trained in the three years of this program). <br />CHAP has produced two photo novellas on topics identified by the lay health advisors (Child <br />Safety in the Kitchen and Childhood Poisoning Prevention Tips) and has received the <br />Outstanding Health Education Project Award, the Gold Project Award, and was a featured <br />finalist far the Glaxo Smith Cline Child Health Recognition Award, Smart Start did not <br />recommend continued funding for this program for FY 03-04. <br />Healthy Carolinians <br />Coordinates health planning and implementation for identified high priority community health <br />issues such as child abuse prevention, teen pregnancy prevention or the prevention of <br />chronic diseases. This program also conducted the quadrennial community assessment for <br />
