EXHIBIT “B” <br />Scope of Services – FY 2024-25 <br />Outside Agency Performance Agreement <br />Agency Name: El Centro Hispano <br />Program Name: Community Support <br />Funding Award: $38,377 <br />Outline how the agency will spend Orange County’s funding award. <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2025. <br />●Increase the number of residents accessing the most appropriate social safety net services <br />●Increase the number of children performing at or above grade level in reading and math <br />●Increase the number of Latino residents who demonstrate new healthy lifestyle behaviors <br />Anticipated Outcomes <br />The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br />County, only (all Towns and municipalities).
