Certified by: _______________________ Title: COO Date: Sept 22 2024 <br />(Provider’s Signature) <br />EXHIBIT “B” <br />Scope of Services – FY 2024-25 <br />Outside Agency Performance Agreement <br />Agency Name: Disability Awarenss Council <br />Program Name: Program for Accessible Resources <br />Funding Award: $15,000 <br />Outline how the agency will spend Orange County’s funding award. <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2025. <br />•Technology and nutrition program to teach people how to search for food and nutrition resources <br />and teach them how to use the internet to find the above resources. <br />•Training Workshops: ADA - addressing challenges that persons with disabilities face in pedestrian <br />safety and job readiness preparedness; Emergency Preparedness - providing educational <br />materials, lectures and safety kits to raise awareness about common safety risks; Affordable <br />Housing - providing prospective tenants' and landlords' rights and responsibilities <br />information and communications tools to readily identify housing discrimination. <br />•Continued use of the new norms including activities being modified including the use of Zoom and <br />other remote services. <br />•Identified a need for a store for technology and disability assistance items. Help to provide <br />additional funds and income for the council. <br />Anticipated Outcomes <br />The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br />County, only (all Towns and municipalities).
