Please download the excel <br />spreadsheet and fill out the demographic data in the table and then upload it <br />with your application. Provide one copy per program that you are requesting <br />funding for. <br />Program Cost <br />This cost per individual must reflect the total program budget divided by the <br />total number of program individuals in this application. <br />Actual Cost 2022-2023 <br />Total Program Cost (Actual 2022-2023) <br />48000 <br />Total Number of Individuals (Actual 2022-2023) <br />610 <br />The Program for Accessible Resources, (PAR) will provide individualized virtual <br />technology training to people <br />with disabilities and improve their livelihoods security and access to digital <br />healthcare, employment and <br />housing services. Usually, 90% of participants are Chapel Hill-Carrboro residents <br />and the remaining are from <br />Orange County. It will address the following needs: <br />• Inadequate Transportation: The ability to provide in-home Technology training <br />with both Zoom and TrainingWare will eliminate the need for transportation <br />services to receive technology training. <br />• Limited Financial Resources: In-home technology training will also eliminate the <br />need for financial resources to travel to meet with an instructor. <br />Docusign Envelope ID: 445D3246-51E8-4212-AB07-F1D18E18D7C6
