APPENDIX 1 <br />SCOPE OF WORK <br />Project Name: Child Planning Conference (CPC) Facilitation <br />A Child Planning Conference (CPC) is a voluntary meeting held within seven (7) days of a legal petition <br />being filed by the Department of Social Services (DSS) against a particular family. The goal of the CPC is <br />to expedite the legal process and service delivery for families who are involved with DSS. A CPC is held in <br />cases in which DSS has petitioned the court, whether or not DSS has taken custody of any children. The <br />CPC is not attended by a judge, however if agreements are made between parties they are entered into <br />a Consent Order, which when signed by a judge becomes part of the DSS record. The CPC often brings <br />legal closure to the case and identifies service and safe placement needs for children early in the <br />process. <br />The core CPC participants include the CPC facilitator, respondent parents, respondent parents' <br />attorneys, DSS social worker(s), DSS supervisor, DSS attorney and guardian ad litem. Other community <br />agency partners that may be included as needed are Health Department representative, Child Support <br />representative, Mental Health representative, School System representative, Substance Abuse <br />representative, Juvenile Justice representative, and Family Violence Prevention Center representative. <br />The Child Planning Conference Facilitator's responsibilities, duties, and tasks include: <br />•Maintaining daily contact with DSS regarding petitions being filed <br />•Notifying CPC team members of upcoming CPCs and providing information regarding the <br />children/family involved, time and location of the CPC <br />•Arranging locations for CPCs and setting up the room <br />•Facilitating CPCs <br />•Distributing/mailing consent orders generated from CPC <br />•Maintaining database of CPC information and outcomes <br />•Compiling, creating, and distributing annual reports to District Court judges, CPC core team, and <br />county partners <br />•Performing other duties deemed appropriate and necessary to the CPC process as requested by <br />the Chief District Court Judge or their designee <br />Docusign Envelope ID: 144E7B0C-8CE1-4B88-AA8E-08C659775648
