Please download the excel <br />spreadsheet and fill out the demographic data in the table and then upload it <br />with your application. Provide one copy per program that you are requesting <br />funding for. <br />Program Cost <br />This cost per individual must reflect the total program budget divided by the <br />total number of program individuals in this application. <br />PPSAT will provide Orange County adolescents and their caregivers (the target <br />population) with comprehensive sexuality education promoting healthy behavior <br />and responsible choices to reduce unintended pregnancy and STI rates.This <br />program will contribute to a network of basic human services and infrastructure <br />that maintains, protects,and promotes the well-being of all county residents <br />(Orange County BOCC Goals and Priorities),and will help youth develop age- <br />appropriate skills and demonstrate new healthy lifestyle behaviors around sexual <br />health (Chapel Hill/Carrboro Obj. 3,Intermediate Result 3.2).The program will <br />ensure a high quality of life for teens and encourage them to be lifelong learners <br />(Orange County BOCC Goals and Priorities). <br />Exhibit A <br />Docusign Envelope ID: 9B12EC66-D23F-408C-BE60-EBCDE242CC54
