Please <br />include specific examples, such as a logic model. <br />Please briefly describe how your proposed programs aligns with evidence-based approaches to addressing <br />human service need(s). <br />RCP improves culturally and linguistically responsive access to public information, <br />resources, and services for refugee and migrant families to increase self-sufficiency <br />and ensure long-term wellbeing. Community Coordinators, who speak Karen, <br />Burmese, Arabic, Spanish, Dari, and Pashto, provide language access and <br />accompaniment for refugee and migrant residents. As a result 150 households (~600 <br />residents) across Chapel Hill will access social services, public benefits, and <br />secure/retain housing. 90% of families will report increased satisfaction with their <br />jobs and housing, and households will increase income and decrease expenses by an <br />average of $1500/household, improving stability. <br />RCP’s culturally and linguistically appropriate onboarding and support enable refugee <br />and migrant residents to meaningfully improve their financial and social well being to <br />achieve self sufficiency and community integration. RCP tracks program participation <br />through a custom-built Salesforce system where every Member has a secure, <br />personal profile. Community Coordinators develop and maintain individual work <br />plans, and progress toward personal goals, from accompaniment through the medical <br />system, to securing new jobs or enrolling in benefits. Data is aggregated into <br />dashboards to review trends and impact over time. Using composite data dashboards <br />and casenote themes, staff conduct impact reviews, discussing trends, new needs, <br />and improvements to make, informed by data and community insights annually. <br />RCP combines participatory community feedback with a holistic understanding of the <br />social determinants of health to guide our programming and work. RCP’s onboarding <br />process and regular member check-ins screen for social determinants of health to <br />identify unmet social needs, improve provider referrals to relevant resources, and <br />improve overall health and financial stability outcomes. Knowing that income greatly <br />influences the stability and social mobility of households, we use relationship based <br />support and culturally relevant appropriate systems to stabilize and impact household <br />income, reviewing and monitoring our successes based on qualitative and <br />quantitative feedback. <br />Docusign Envelope ID: 9A1CFCD5-41FF-4C02-9172-BC499052FC3A
