The health assessment must have been completed within 12 months prior to the date of entry to public school. If the school hasn't received the health assessment form and a record of completed immunizations by the 30th calendar day from the first day of a child's attendance, the principal is required by law to exclude the child from school on the 31 st day until such evidence has been presented by the parent or guardian. Many children come to the health department for their school health assessment and immunizations during that 30-calendar day period before school exclusion. If an age- appropriate child health preventive health visit cannot be performed during time allotted for the School Health Assessment, the NC Division of Child and Family Well-Being, Whole Child Health Section allows for a Stand-Alone School Health Assessment at a fixed rate to improve access for children and adolescents to avoid exclusion from school.
