5 <br /> SUBMISSION INFORMATION <br /> You must submit your application and supplemental documents using the on-line OpenGov portal. <br /> Question should be sent to: <br /> - Town of Carrboro: humanservices@carrboronc.gov <br /> - Town of Chapel Hill: humanservices@townofchapelhill.org <br /> - Orange County: outsideagencies@orangecountync.gov <br /> - Label all attachments in the following format: A. (Your Agency Name) (Your Document <br /> Name) <br /> o For example: A. ABC Nonprofit Financial Audit <br /> Human Services Awards <br /> The Human Services Award program is for funding requests to support vital community services <br /> throughout Orange County. This is a very competitive process and typically awards are made to <br /> organizations that have been in operation for at least one year. <br /> Eligible proposals: <br /> ■ Are from established 501c3 organizations, with current documentation of their nonprofit <br /> status. <br /> ■ Are in good standing with the Towns and County and do not have unresolved performance <br /> or compliance issues. <br /> ■ Align with at least one of the three strategic objectives of the Towns Human Services Result <br /> Framework (for Towns of Chapel Hill and Carrboro only). <br /> ■ Request $2,000 or more (for Town of Chapel Hill only). <br /> ■ Are submitted on time and are complete. <br /> Chapel Hill & Carrboro Applicants Only: <br /> Small Awards <br /> The Small Award Program is for funding requests at or below $5,000. <br /> Applicants of the Small Award program will submit all of the information required in the <br /> application. Agencies requesting $5,000 or below will not be required to attend the agency hearing; <br /> attendance is optional. The hearing is required for requests over $5,000. <br /> Based on the results of the Racial Equity Assessment Lens (REAL) of the Humans Services process, <br /> staff will allow up-front payments for certain applicants. The intention of providing up-front awards <br /> rather than on a reimbursement basis is to ensure smaller, newer, and potentially less established <br /> organizations can serve community needs. Only agencies requesting at or below $5,000 will be <br /> eligible to receive an up-front award of the total amount, pending recommendation from the <br /> HSAB. Funding parameters include: <br /> 1. Performance agreement (same as other agencies) <br /> 2.
