Description of Work <br />Fire Sprinkler Service & Repair Estimate <br />Scope of Work: Dry System did operate when tested. Piston inside Valve Seized shut. Will not allow water to flow. <br />Replace Dry Valve Victaulic with a Tyco DPV-1. Emergency Service Request <br />1. Replace Dry Valve <br />2. Retest Dry Valve <br />Impact Fire Services appreciates the opportunity to provide you with the following proposal for services and repairs. All services will be <br />completed by qualified personnel in accordance with NFPA 13 and 25. This proposal includes all materials, labor, and permits for the <br />work listed. Additional work will be quoted separately upon request. <br />All services are performed during normal work hours Monday through Friday 7:00 am to 5:00 pm (excluding holidays), unless otherwise <br />agreed upon in advance. All work listed on the proposal must be accepted and scheduled for the same date to maintain the proposal <br />pricing. The owner/owner's representative shall place alarm systems on test and restore upon completion of work. <br />EXCLUSIONS: <br />Any Additional Work NOT Specifically Listed in the proposal. <br />No Draining of Low Point Drains Other Than at the Time of The Service <br />Over Time And/or After-Hours Costs Weekends And/or Holidays <br />Any Fire Alarm and/or Electrical Work of Any Kind <br />Lift rental to perform the NFPA-25 required internal pipe investigation and/or service work <br />No draining of low point drains other than at the time of service. <br />Overtime, after normal working hours, weekends, and/or holiday hours are not included. <br />Any Fire Alarm and/or electrical work of any kind are not included. <br />Cutting out and/or replacing sheetrock/hard ceilings/hard walls or cutting of access panels. <br />Any unknown or hidden damage prior to or during scope of work. <br />Replacement of CPVC and/or polybutylene piping that may break when removing/replacing the fire sprinkler heads. <br />LIMITATION OF LIABILITY <br />Impact Fire Services does not assume any responsibility for failure to operate or faulty operation as such failures may be due to causes <br />beyond our control such as malicious tampering by the owner or third parties, accidents, failure to perform required <br />housekeeping/maintenance, failure to perform required modifications, acts of God, etc. After any Inspection or Maintenance, the fire <br />protection system will be placed back into service with all components (Valves, Switches, Detectors, etc.) in operational condition by <br />Impact Fire Services and henceforth the owner is responsible for further care and maintenance as described in the applicable codes and <br />standards. It is understood and agreed that Impact Fire Services is NOT an insurer and is NOT warranting or guaranteeing the adequacy, <br />performance, or life expectancy of any structure, item, component, or system in the building. Impact Fire Services expressly waives all <br />warranties, including any warranty for fitness for a particular product, connected with its work or the systems it implements, aside from <br />those written warranties provided to the owner as a part of the contract for the work performed if any.
