4 <br />Deliverables <br />Tier One Executive Team – Communications Strategy (40 hours) <br /> <br />1) Develop and conduct a Conflict Management Workshop reviewing theoretical <br />models of conflict resolution and incorporating practical application. <br /> <br />2) Use a model to reshape expectations of engagement requiring weekly <br />commitments. <br /> <br />3) Create and adopt a communication plan that includes dissemination to <br />organization. <br /> <br />Tier Two Clinic – Follow up from Clinic Retreat recommendations held <br />on June 21, 2023 (40 hours) <br /> <br />1) Conduct (15) 30-minute clinic interviews to reengage <br />2) Finish the following sections of charter <br />• Activity <br />• Building our shared values <br />• How we make decisions <br />• Limitations or constraints of team <br />• Major peeves <br />• How we will resolve conflict <br />3) Conduct a Clinical Needs Assessment using PLANNING TOOLKIT FOR USING <br />CDC’s Recommendations for Providing Quality Services – gets to purpose and <br />defining performance - 8 worksheets plus action plan. <br />4) Identify positions that require a standardization of work. <br />5) Reinitiate the WTF Committee to work on scheduling issue implementing the <br />following recommendations from retreat: <br />• Establish a calendar of meetings for 2024 -25 <br />• Use a required agenda – template in appendices <br />• Define roles – at a minimum, require the following: facilitator, note taker, <br />accountability role. <br /> <br /> <br /> <br /> <br /> <br />DocuSign Envelope ID: 4BDE5E37-EDF3-4CCD-A393-1A08FE321B16
