Revised 01/24 <br /> <br />NORTH CAROLINA <br />CONTRACT AMENDMENT <br />ORANGE COUNTY <br /> <br /> <br />THIS CONTRACT AMENDMENT (“Amendment”) is made and entered into this 20th day of May, 2024 by and between <br />ORANGE COUNTY (hereinafter referred to as “County”) and University of North Carolina at Chapel Hill (hereinafter <br />referred to as “Provider”). <br /> <br /> <br />WITNESSETH: <br /> <br />THAT WHEREAS, the County and Provider entered into a contract dated December 1, 2023, (hereinafter the “Original <br />Agreement”), for the provision of services for support programs that serve persons with Opioid Use Disorder (OUD); and <br /> <br />WHEREAS, the County and Provider desire to amend the Original Agreement while keeping in effect all terms and <br />conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. <br /> <br />NOW THEREFORE, for and in consideration of the mutual covenants and agreements made herein, the parties agree to <br />amend the Original Agreement as follows: <br /> <br />1. In order to ensure the completion of the Services identified in the term of the Original Agreement is amended <br />to reflect an end date by which all Services shall be completed of December 31, 2024. <br /> <br />2. Exhibit A to the Original Agreement is amended by changing and adding the following tasks and services to <br />the Services to be provided by the Consultant: Metric reporting and the narrative story is required by July 30, <br />2024 for December 2023 through June 2024 and January 15, 2025 for July 2024 through December 2024. <br /> <br />3. Article 3., Section a. is amended to reflect additional invoicing dates of October 10, 2024 for service period <br />July 1, 2024 through September 30, 2024 and January 15, 2025 for service period October 1, 2024 through <br />December 31, 2024. A metric report is no longer due April 15, 2024. <br /> <br />4. Except for the changes made herein, the Original Agreement shall remain in full force and effect to the extent <br />it is not inconsistent with this Amendment.
