Mold <br />samples are hand delivered to a professional third-party accredited lab (AIHA-LAP), meeting all requirements of the <br />ISO 17025:2017 Standards. Air samples are collected using professional and calibrated air sampling pumps. <br />•Data collection of Relative Humidity (RH) and Carbon Dioxide (CO2) throughout the sampling areas. <br />•Priority report preparation, the report will explain the lab results. <br />•Custom forms or handouts for staff/occupants prepared by ESG to explain IAQ concepts. <br />•Up to 2 presentations/meetings a year for staff/occupants. <br />•Phone consultation with a dedicated Indoor Environmental Professional (IEP) regarding the >indings. See below for <br />details. <br />•Certi>icate for display that shows compliance with ESG’s Indoor Air Wellness Program. <br />•The 2 buildings included in the program are: Emergency Services and Hillsborough Commons. The price above was <br />custom designed for the 2 buildings after creating diagrams of ideal sampling locations for each building. These <br />sampling diagrams can be provided to you if requested. Any change or addition to these buildings will need to be done <br />with a new pricing structure and agreement. <br />Termination of Service Agreement. Notwithstanding any other provision of a Service Agreement, a Customer may <br />terminate any Service Agreement awarded on a no fault basis at any time by giving the Service Provider 120 days’ notice in <br />writing. The Customer may extend the period of notice at any time before it expires subject to agreement on the level <br />of services to be provided by the Service Provider during the period of extension. In the event of early termination of this <br />contract (e.g. canceling quarterly sampling after only two air sampling events), discounted services that were performed as <br />part of the program may be billed at their non-discounted prices. <br />Use of Reports. The reports provided by ESG is intended as a record of conditions observed by ESG at the time of assessment. <br />While this material is helpful to a contractor, ESG requires proper notice and compensation for any unanticipated services that <br />were not originally included in the cost of the service. This includes all legal-related services such as depositions, trials, legal <br />preparation, and legal consultation. ESG can provide expert testimony services and prepare reports for litigation purposes with <br />proper notice and appropriate compensation.
