Revised 01/24 <br />1 <br /> [Departmental Use Only] <br /> TITLE WCOB Elevator Repair <br /> FY 2023/2024 <br />NORTH CAROLINA <br /> SERVICES AGREEMENT NO RFP/RFQ <br /> <br />ORANGE COUNTY <br /> <br /> This Services Agreement (hereinafter “Agreement”), made and entered into this 8th day of <br />May, 2024, (“Effective Date”) by and between Orange County, North Carolina a political <br />subdivision of the State of North Carolina (hereinafter, the "County") and Otis Elevator <br />Company, (hereinafter, the "Provider"). <br /> <br />WITNESSETH: <br /> <br /> That the County and Provider, for the consideration herein named, do hereby agree as <br />follows: <br /> <br />1. Services <br /> <br />a. Scope of Work. <br /> <br />i) This Agreement is for services to be rendered by Provider to County with respect <br />to (insert type of project): Emergency Work:The existing power unit will be <br />replaced with a new power unit. The new power unit consists of a positive <br />displacement pump, motor, integral 4-coil control valve, oil tank and muffler. The <br />pump and motor are submerged and are mounted to the tank with rubber isolators to <br />reduce vibration and noise. A muffler is provided to dissipate pulsations and noise <br />from the flow of hydraulic fluid. The valve consists of up, up leveling, down and <br />down leveling controls along with manual lowering and a pressure relief valve. <br />ii) Oil disposal is not included. If any additional work is needed, we will provide <br />another proposal. The scope proposed herein represents the entire scope that Otis is <br />contracted for, if additional work is required by others to allow for completion of <br />this work and/or for the inspection to occur, that work is not included. Tax is not <br />included. Material provided shall be installed in accordance with the ASME A17.1 <br />Safety Code for Elevators and Escalators. The customer will be responsible for <br />paying local inspection fees if applicable. A representative will contact you to <br />schedule the work. All work will be performed during regular working days and <br />hours of the Elevator Trade unless otherwise specified above.
