Certificates and Certifications Submittals: Submit a statement that includes signature of <br /> entity responsible for preparing certification. Certificates and certifications shall be <br /> signed by an officer or other individual authorized to sign documents on behalf of that <br /> entity. Provide a notarized signature where indicated. <br /> 2.
