Facility / Date: Charles House Winmore
August 9, 2016

Resident Services Yes
No
N/A
Comments/Other Observations (please
number comments)
15. Were residents asked their preferences or opinions
about the activities planned for them at the facility?
Yes 16. Due to the level of dementia for these
residents, they do not manage their own
finances. All residents have family in the area
and they are visited by their families who buy
extras for them and help with choices.
Everyone seemed quite comfortable about the
situation.

18. Residents are unable to communicate on
the phone.

19. Heather thought it would be great if
religious groups, civic or volunteer groups
came by and visited with the residents. That is
not not taking place.

20. The residents cannot have a council due
their dementia but the families are quite
active.
15a. Was a current activity calendar posted in the facility? Yes
15b. Were activities scheduled to occur at the time of your
visit actually occurring?
Yes
16. Do residents have the opportunity to purchase personal
items of their choice using their monthly needs funds?
Yes
16a.Can residents access their monthly needs funds at their
convenience?
Yes
17. Are residents asked their preferences about meal &
snack choices?
Yes
17a. Are they given a choice about where they prefer to dine? Yes
17b. Did residents express positive opinions regarding their
dining experience?
Yes
17c. Is fresh ice water available and provided to residents? Yes
18. Do residents have privacy in making and receiving
phone calls?
N/A
19. Is there evidence of community involvement from other
Civic, volunteer or religious groups?
No
20. Does the facility have a functioning: Resident’s Council?
Family Council?
N/A

Areas of Concern Exit Summary
Are there resident issues or topics that need follow-up or
review at a later time or during the next visit?
None noted during this visit. We will try to visit with
residents alone as well at future visits.

Discuss items from “Areas of Concern” Section as well as
any changes observed during the visit. Give summary of
visit with Administrator or SIC. Does the facility have
needs that the committee or community could help address?
This is a new facility and is extremely clean, modern and
to all appearances well run. The residents were finishing
lunch as we arrived. After touring the facility we sat with
the residents, Heather and the assistant, and visited for
about 45 minutes with them all. All residents had trouble
speaking and understanding but they appeared to have a
good relationship with Heather and staff and actively tried
hard to communicate. The residents indicated they would
like us to come again and “chat” with them.
