26 <br /> Outcome Reporting: <br /> All related data pertaining to the PORT program will be reported as part of the quarterly Orange County <br /> Peer Review Committee. De-identified quarterly data will be provided to the Opioid Advisory Committee. <br /> Duration of Service <br /> We are requesting the utilization of the Orange County, NC allotment of Federal opioid settlement funds <br /> to provide three years of the outlined service. By the final year of service, program and surveillance data <br /> will be utilized to determine ongoing need and sustainability of services. <br /> Proiect Implementation Timeline <br /> PHASE I(months 0-4) <br /> This initial phase will consist primarily of program development, staffing, logistics and training. During <br /> this phase, MOUs will be established with partnering OBOTs, OTPs, Orange County Departments, <br /> emergency departments and local organizations. Internal protocols and policies will be updated to reflect <br /> the availability of this service, trainings will be created and conducted with all potential partners and <br /> stakeholders,and program processes and logistics will be executed.During program development,feedback <br /> will be solicited from stakeholders and individuals with lived experience. All physical items required to <br /> provide service will be obtained during this phase. <br /> PHASE II(months 5-12) <br /> PORT program implementation will begin with this phase and will include responding to opioid overdose <br /> incidents,providing services to inbound referrals and executing all program processes. Outreach efforts to <br /> ensure community awareness of program and services will take place during this phase. Project staff will <br /> begin quarterly reporting to partners including the Opioid Advisory Committee. <br /> PHASE III(years 2 &3) <br /> Throughout phase three, the focus will be on program continuation, adjustments to accommodate <br /> community needs that arise, and preparation for sustainability of the program. In approaching the final year <br /> of service,program data will be further analyzed to determine ongoing sustainability of services. <br />
