If unable to reach, provide email containing applicable infiormation of <br /> intervention and flagged as `high importance'. <br /> Alternative Patient Origin: <br /> LANTERN: <br /> In the event that a client is referred from Criminal Justice Resource Department's LANTERN Project for a gap in treatment <br /> access at jail release,that person may be continued on their prescribed dosage of buprenorphine for up to 7 days from the last <br /> available dose of the medication supplied at release.This pathway does not require a COWS scale to be performed. <br /> Upon receiving referral from LANTERN, the Community Paramedic Coordinator is responsible for confirming the <br /> following prior to initiation of MAT: <br /> • Patient's prescribed Buprenorphine dosage at release from Detention Center <br /> • Patient is active client of LANTERN and is being connected to ongoing treatment by LANTERN staff <br /> Buprenorphine can not be administered without both of the previous criteria in place. <br /> 2 <br />
