Certified by: _______________________ Title: __________________________ Date: ____________ <br /> (Provider’s Signature) <br /> <br /> <br />EXHIBIT “B” <br /> <br />Scope of Services – FY 2023-24 <br />Outside Agency Performance Agreement <br /> <br />Agency Name: EmPOWERment Inc. <br />Program Name: Affordable Rental Program <br />Funding Award: $60,000 <br /> <br />Outline how the agency will spend Orange County’s funding award. <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2024. <br />• Decrease the number of evictions due to nonpayment of rent in Orange County by 20 <br />• To increase the livelihood and security of Orange County residents by prviding supportive services <br />that will aid in meeting some of their basic needs. <br />• To provide housing for the most vulnerable population: homeless, disabled, housing choice voucher <br />holders, and the veteran population. <br /> <br />Anticipated Outcomes <br />The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br />County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within <br />that measure’s description or for an earlier performance measure. <br />Performance Measures Anticipated <br />Results <br />Tenants who are experiencing rental hardships are encouraged to attend counseling with <br />EI’s HUD Certified Housing Counselors. These sessions have been created to include <br />financial literacy, budgeting and information about the eviction process. Once the classes <br />are completed, EI’s counselors will work with the tenant on creating a payment plan <br />acceptable to the landlord for the tenants to avoid the eviction process. <br />30 <br />Throughout the year Orange County residents are given the opportunity to receive <br />supportive services from the agency in the following ways: emergency utility assistance <br />through the M.O.M fund; food and meal distributions; clothing and toy give aways; and <br />gift card distributions. <br />500 <br />Collaboration with other nonprofits and local government agencies to reach underserved <br />and disenfranchised populations. <br />4 <br /> <br />Expense Description Amount <br />Operations for Affordable Rental Program $60,000 <br /> <br /> <br /> <br /> <br /> <br />Property Manager 10/27/2023 <br />DocuSign Envelope ID: 8E979ED6-1D8F-4C24-8DEC-03C92D7020EE
