B <br /> DOA-734 <br /> (revised 1/99) <br /> July 1, 2002 through Jane 30, 2003 <br /> Home and Community Cam Block Grant far Older Adults <br /> Community Service Provider <br /> Standard Assurances <br /> Couary of Social Services agoms to Myrtle services through the Home and <br /> (Name of Provider) <br /> Community Cara Black Grem, as specified on the provider Services Summary (DOA-732) <br /> in accordance with the following: <br /> 1. Services shall be provided in accordance with requirements set botch a : <br /> a) The County Funding Plan; <br /> b) The Division of Aging Home and Community Care Block Grant Procedures <br /> Maand for Community Service Providers; and <br /> c) The Division of Aging Services Standards Manual, Volumes I through IV. <br /> Community service providers shad monitor any contracts with providers of Block <br /> Grant services and tale appropriate measures to ensure that services are provided in <br /> accordance with the aforementioned documents. <br /> 2. Priority .shin he given to providing services or these older persons with he greatest <br /> accurate or social made, with pacticalar attention in low-income minority <br /> ladividuals. The service needs of low-mcame minority elderly will be addressed in <br /> the counter specified on the MetWtlolo a Address Service Needs rf Law-Income <br /> Minority Elderly facing, (DOA 733). <br /> 3. The following service authorization activities will be carried out Or conjunction with <br /> an services provided through the Block Grans <br /> a) Eligibility determination; <br /> b) Client intake/registration; <br /> i) Client assesemmUreasseeements and generly visits, as appropri ce; <br /> d) DetermWng The amount of services to be received by the client; and <br /> e) Reviewing cost sharing/volummy contributions policies with eligible clients. <br /> 4. All licenses,permits, bow%, and insurance necessary for carrying out Block Grant <br />
