<br /> <br />Program information P a g e 19 of 28 <br />Community Impact Award <br />If you are applying for the Town of Chapel Hill’s Community Impact Award, please provide <br />responses to the questions below. All other applicants, please skip these questions. (Responses <br />should not exceed 100 words per question) <br /> <br />1. Please describe the impact the proposed programs will have on the target population. <br />Please include specific quantitative and qualitative data in your response. <br />The delivery of healthy meals accompanied by friendly check-ins, improves health, <br />alleviates hunger, and reduces isolation. Nationally, MOW recipients have reduced <br />emergency room visits (20%), hospitalization rates (36%) and nursing home usage (30%). <br />They report healthier eating, improved mental health, and reductions in the rate of falls, <br />feelings of loneliness, and anxiety. Locally, recipients agree/strongly agree that <br /> (93%) receiving meals on wheels helps me to eat more nutritious food. <br /> (90%) receiving meals on wheels has improved my health. <br /> (100%) receiving services helps me remain in my home. <br /> (87%) receiving services reduces my need for in-home care. <br /> <br />2. What methods/tools will your organization use to evaluate the proposed program’s <br />effectiveness? Please include specific examples, such as a logic model. <br />Program effectiveness is based on: <br /> satisfaction with service <br /> connectedness with volunteers <br /> impact of MOWOCNC service on nutrition, health, and ability to age in place <br /> as well as impact on reducing food insecurity and social isolation and loneliness <br /> <br />Quantitative and qualitative data from recipient (bi-annually), volunteer, board member and <br />staff (annually) surveys along with donor, volunteer, and community focus groups (tri- <br />annually) were analyzed and integrated into our new strategic plan. The data is continually <br />utilized and updated along with the daily direct feedback from volunteers and recipients to <br />consistently evaluate program effectiveness and impact. <br /> <br />3. Please briefly describe how your proposed programs aligns with evidence-based <br />approaches to addressing human service need(s). <br /> <br />The core of our service is the delivery of consistent, nutritious meals and friendly <br />check-ins. Access to good nutrition is critical as older adults are often at risk of <br />malnutrition given the biological, social, economic, and functional challenges that often <br />accompany aging and limit a senior’s ability to acquire, prepare and consume nutritious <br />foods. Research suggests that receiving MOW services reduces food insecurity rates <br />by up to 28%.
