Revised 04/23 <br />3 <br />3. Basic Services <br /> <br />a. Basic Services. The Services to be rendered pursuant to this Agreement are as follows <br />(fully describe services to be provided): <br />b. According to the North Carolina General Statutes 130A-415, a dead body shall be <br />deemed unclaimed if the following occurs: no individual notifies the person in <br />possession of the dead body within 10 days of the date of death that the individual <br />wishes to dispose of the dead body; all individuals who have expressed interest in <br />arranging for the disposition of the dead body have ceased communication with the <br />person in possession of the body for five days, at least 10 days have passed since the <br />date of death, and the person in possession of the dead body has used reasonable efforts <br />to contact all individuals interested in arranging for final disposition. <br />c. The County will contact the Provider via telephone and will fax the Standard Cremation <br />Authorization Form along with any other relevant information. <br />d. The County will provide any information known to be relevant to securing and <br />completing a death certificate. <br />e. Provider will pick up and transport unclaimed body. <br />f. Provider will secure information for death certificate and ensure that death certificate is <br />filed with appropriate entity. <br />g. Provider will cremate unclaimed bodies according to State rules and regulations. <br />h. Provider will provide County with a copy of the death certificate is requested. <br />i. The County will render payment of $650 for services performed. Payments will be made <br />within 30 days of receiving invoice from Provider. <br /> <br />4. Duration of Services <br /> <br />a. Term. The term of this Agreement shall be from July 1, 2023 to June 30, 2024. <br /> <br />b. Scheduling of Services. <br />i) The Provider shall schedule and perform its activities in a timely manner. <br /> <br />ii) Should the County determine that the Provider is behind schedule, it may require <br />the Provider to expedite and accelerate its efforts, including providing additional <br />resources and working overtime, as necessary, to perform its services in <br />accordance with the approved project schedule at no additional cost to the <br />County. <br /> <br />iii) The Commencement Date for the Provider's Basic Services shall be July 1, 2023. <br /> <br />5. Compensation <br /> <br />a. Compensation for Basic Services. Compensation for Basic Services shall include all <br />compensation due the Provider from the County for all services satisfactorily (as <br />determined by the County) performed pursuant to this Agreement. The maximum <br />amount payable for Basic Services shall not exceed Ten Thousand Dollars ($10,000). <br />Payment for satisfactorily performed Basic Services shall become due and payable <br />within thirty (30) days of Provider properly invoicing County.
