Revised 04/23 <br />3 <br />3. Basic Services <br /> <br />a. Basic Services. The Services to be rendered pursuant to this Agreement are as follows <br />(fully describe services to be provided): <br />b. Carolina Outreach will provide one licensed therapist or two half time licensed therapists <br />who will each offer at least 12 one hour therapy slots per week to adults involved with <br />DSS. <br />c. Sessions will be held at the Hillsborough and/or Chapel Hill DSS office location in a <br />space provided by DSS that supports confidentiality or via telehealth. <br />d. Caroline Outreach therapist will create a consistent weekly schedule most likely <br />covering four full days and will make DSS staff aware of this schedule. Therapist will <br />also alert DSS point of contact of any changes to schedule due to illness, vacation, or <br />desire to change days. <br />e. Individuals referred for therapy through this co-located program will be expected to sign <br />Carolina Outreach Consent to Treat as well as Release of Information documentation in <br />order for treatment to begin as well as attendance compliance and/or treatment progress <br />to be shared with DSS. Client referred by DSS will be expected to sign at a minimum a <br />release for Carolina Outreach to share attendance. <br />f. Therapist will complete a Clinical Assessment at first appointment and then make <br />recommendations for treatment. If Outpatient Therapy is the clinically appropriate level <br />of care, Carolina Outreach therapist will create a treatment plan with the client and offer <br />individual and/or family therapy for a length of time that is appropriate based on <br />diagnosis and symptom presentation. If client referred is in need of a higher level of <br />care, therapist will make an attempt to refer to the appropriate service as is possible, <br />considering funding source availability. <br />g. If referred client does not engage at all after referral or initial assessment or has four late <br />cancellations, or no shows, within a three month period, client may be terminated from <br />outpatient therapy services. <br />h. Carolina Outreach will send quarterly data reports to Orange County DSS to include data <br />points agreed upon by Orange County DSS Human Services Program Manager and <br />Carolina Outreach therapist and supervisor. <br />i. DSS will refer clients through the online referral page on the Carolina Outreach website. <br />Carolina Outreach therapist will make initial contact to set up the first appointment. <br />Ongoing, clients will be provided a contact number to communicate directly with the <br />assigned therapist in regards to appointments. <br />j. Additionally, the therapist can refer a client to Carolina Outreach for psychiatry <br />consultation/medication management if this is an identified need. If the client has no <br />income and IPRS funds are available, IPRS funds will be utilized. In all other instances, <br />DSS will use funds to cover the costs of these appointments. Any clients who present as <br />a conflict of interest for the treating therapist will be referred to another outpatient <br />therapist. <br /> <br />4. Duration of Services <br /> <br />a. Term. The term of this Agreement shall be from July 1, 2023 to June 30, 2024. <br /> <br />b.
