Efforts made towards restoration of rights, identification of a successor <br />guardian, or the possibility of limited guardianship. <br />4. The Contractor is required to meet all goals and outcomes listed in Attachment O. <br /> <br />Social Services Responsibilities: <br />1. Provide a Supervisor who will manage the programmatic area of the contract. <br /> The Supervisor will: <br />a. Be the point of contact for the Contractor. <br />b. Maintain communication with Contractor. <br />c. Keep Contractor informed of any policy changes. <br />d. Monitor the Contract. Monitoring includes, but not limited to: review <br />of monthly progress reports forwarded by Contractor, review and <br />approval of monthly expenditures, random sampling of invoices and <br />payments, may randomly conduct announced and unannounced site <br />visits, annual monitoring to include review of records, eligibility, <br />attendance/participation, expenditures and review of contract <br />procedures. <br />e. Attend scheduled meetings with Contractor to discuss Contract <br />programmatic and/or administrative matters. <br />2. Initiate and follow through with the transition of GOP responsibilities via the <br />Clerk of Superior Court in Orange County, North Carolina as well as new <br />appointments. <br />3. Up to ten (10) wards may be assigned to Contractor throughout the term of the <br />contract. Prior to Contractor assuming oversight, Supervisor will: <br />a. Select wards; <br />b. Assign cases, and <br />c. Provide an overview and discuss each case, providing all <br />appropriate information. <br />4. In the event there is a decrease in the number of wards due to death, an <br />appointment of GOP successor or the restoration of rights, Supervisor will <br />assign a replacement to maintain the agreed upon number of cases as <br />identified above and initiate the court process to make the Contractor the <br />successor guardian. <br />5. Wards receiving Medicaid will continue unless there is a determination of <br />ineligibility. <br />6. Inform Contractor of any changes to the Guardianship regulations/laws. <br /> <br /> <br />C. Rate per unit of Service (define the unit): <br /> <br />1. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br /> <br />A maximum allowable rate of $275 per assigned ward per month, which includes <br />Guardianship Case Management Services.
