Explanation of Services to be provided and to whom (include SIS Service Code): <br />1. As the assigned Guardianship of Persons (GOP) for Wards referred and <br />assigned by Social Services, Contractor will: <br />a. Attend court hearing(s) and any subsequent proceedings including <br />petitioning for restoration and successor guardians for each new Social <br />Services referral where GOP responsibilities will be transitioned from <br />Social Services to Contractor. <br />b. Provide Adult Guardianship Case Management Services. <br />c. Maintain a file which will include the Letter of Appointment in each <br />record. <br />d. Maintain ongoing notes and documentation related to all services <br />performed pursuant to this Contract. <br />e. Conduct face-to-face holistic assessment every 90 days for each Social <br />Services Ward. Additional visits may be warranted based on needs of <br />the Social Services Ward and will be performed by the Contractor at <br />no additional cost to Social Services. <br />f. Conduct at least monthly telephone contact with each Ward and other <br />relevant service providers. <br />g. Meet or satisfy all GOP responsibilities. <br />h. Comply with all GOP duties as set forth in Chapter 35A of the North <br />Carolina General Statutes. <br />2. Ensure all employees providing Guardianship Case Management Services <br />maintain at all times all necessary certifications during the term of this <br />Contract. <br />3. Within thirty (30) days of the anniversary date of the Service Provider being <br />named GOP for each new Social Services referral, submit a status report of <br />each Ward directly to the Orange County Clerk of Superior Court, with a copy <br />forwarded via email to the Social Services Adult Services Supervisor. The <br />report must include the following information for each Social Services Ward: <br />a.
