Revised 04/23 <br />3 <br /> <br />a. Basic Services. The Services to be rendered pursuant to this Agreement are as follows <br />(fully describe services to be provided): <br />b. Provider will provide up to 50 diapers per child per month (sizes newborn to 5 only) or <br />30 pull-ups per child per month for up to 150 participants per month. As available, the <br />Provider will provide diapers of other sizes. <br />c. Diapers will be picked up at the Provider's location by the County for use by participants <br />engaged in Social Services' programming, including, but not limited to, the Community <br />Response Program, the Adolescent Parenting Program, and Child Protective Services. <br />d. As available, the Provider's toddler toilet training classes will be open to Social Services' <br />participants. <br />e. Provider will track percentages of families reporting that receiving diapers allows them <br />to experience the outcomes of: reduced parental stress; increased ability to afford food, <br />pay bills, or purchase other non-food hygiene products; increased child health and <br />happiness; and increased work and childcare attendance. <br />f. Provider will provide 870 adult incontinence products to up to 29 low-income adults and <br />8,500 period products to up to 425 menstruating individuals per month. <br />g. Hygiene products will be picked up at the Provider's location by the County. Provider <br />will work with Social Services to identify individuals in need throughout the county. <br />Hygienge products may be distributed to participants engaged in Social Services' <br />programming, low-income adults receiving eldercare services through DSS/Aging <br />partnership, and/or recipients of other community services. <br />h. Provider will track the number of individuals served, products distributed, and percent of <br />recipients who report increased livelihood security by accessing social safety net <br />services which enable them to attain stability and thrive. <br />i. Provider will submit program progress reports, fiscal reports, and invoices on a quarterly <br />basis. The County will reimburse Provider for administrative costs required to provide <br />the services described in this contract up to the budgetary limits of the contract <br />allotment. Costs include: compensation, rent, utilities, supplies, and equipment. <br />j. The County has allocated to Provider Ten Thousand dollars ($10,000) to operate the <br />diaper distribution program and Ten Thousand dollars ($10,000) to operate the hygiene <br />product distribution program. <br />k. Provider shall be paid in four equal installments in the amount of Five Thousand dollars <br />($5,000). The first payment in contingent upon receipt of the agency's performance <br />agreement; the remaining payments are contingent upon receipt of the request for <br />reimbursement and related supporting documentation. <br />l. The County's obligation to make the quarterly payments is contingent upon receipt of <br />progress reports, which show satisfactory progress toward completion of performance <br />measures and an accounting of expenditures as detailed in 3(i). <br /> <br />4. Duration of Services <br /> <br />a. Term. The term of this Agreement shall be from July 1, 2023 to June 30, 2024. <br /> <br />b.
