FSA's target population includes at-risk families who often face great inequities, <br /> including not having a medical home for primary care. FSA staff aim to improve the number of families with a <br /> medical home. Anecdotally,staff know that among FSA families,there are more children than adults with medical <br /> homes. Therefore the number of families with medical homes will be tracked over time as a measurable target, <br /> using the REDCap electronic data capture system. <br /> 41 <br />
