I have <br /> read the Title VI Program and I am committed to ensuring that no person is excluded from participation <br /> in or denied the benefits of OCTS programs, policies, services and activities on the basis of race, color, <br /> national origin, sex, age, creed (religion), or disability, as provided by Title VI of the Civil Rights Act of <br /> 1964 and related nondiscrimination statutes. <br /> Signature Date <br /> Implementation <br /> • Periodically, but not more than once a year, employees and representatives will receive, <br /> review and certify commitment to the Title VI Program. <br /> • New employees shall be informed of Title VI provisions and expectations to perform their <br /> duties, accordingly, asked to review the Title VI Program, and required to sign the <br /> acknowledgement form. <br /> • Periodic review of operational practices and guidelines by the Title VI Coordinator to verify <br /> compliance with the Title VI Program.
