Attachment 2 4 <br />Senior Citizen Health Promotion Program <br />Grant Project Ordinance <br />Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br />13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br />hereby adopted. <br />Section 1. This project authorized is the Senior Citizen Health Promotion Program to develop <br />a comprehensive and coordinated County health promotion and disease prevention <br />program for citizens involving a partnership of public agencies, University Health <br />Affairs and private groups. <br />Section 2.The officers of the County are hereby directed to proceed with the grant project in <br />accordance with the mutual cooperation agreement between the County and the <br />University of North Carolina at Chapel Hill, as approved by the Board of County <br />Commissioners on June 30, 1997. <br />Section 3.The following revenue is anticipated to be available to complete this project <br />Grant Funds (1994-95) <br />Grant Funds (1995-96) <br />Grant Funds (1996-97) <br />Grant Funds (1997-98) <br />Fees for Service (1997-98) <br />Grant Funds (1998-99) <br />Fees for Service (1998-99) <br />Grant Funds (1999-00) <br />Fees for Service (2000-01) <br />Grant Funds (2000-01) <br />Grant Funds (2001-02) <br />Grant Funds (2002-03) <br />Fees for Service (2003-04) <br />Grant Funds (2003-04) <br />Total Revenue <br />Section 4. Reimbursement requests should be made to the grantor agency in <br />an orderly and timely manner.
