Certified by: _______________________ Title: __________________________ Date: ____________ <br /> (Provider’s Signature) <br /> <br /> <br />EXHIBIT “B” <br /> <br />Scope of Services – FY 2022-2023 <br />Outside Agency Performance Agreement <br /> <br />Agency Name: The Women's Center, Inc. dba Compass Center for Women and Families <br />Program Name: Self-Sufficiency Services <br />Funding Award: $104,281.00 (divided across 3 program areas) <br /> <br />Outline how the agency will spend Orange County’s funding award. <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2023. <br /> Financial Education: Assists individuals and families to successfully manage their finances, reduce <br />debts, build assets and meet other short-and-long-term goals in individual sessions and workshops. <br /> Career Coaching: Provide career counseling and support in individual advising and workshops. <br />Services include help with resume writing and interviewing skills, job search skills, assessment of <br />career and educational opportunities, development of job search process and a variety of <br />workshops. <br /> Legal Information Services: Provides accessible, understandable, and respectful free legal <br />information to clients who cannot afford legal services. Services include telephone appointments <br />with attorneys, in-person appointments with supervised law students, legal workshops, and legal <br />clinics. <br /> <br />Anticipated Outcomes <br />The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br />County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within <br />that measure’s description or for an earlier performance measure. <br />Expense Description Amount <br />Personnel, programmatic, and overhead expenses.
