Outside Agencies/Human Services <br /> <br /> <br />Community Impact Award <br />If you are applying for the Town of Chapel Hill’s Community Impact Award, please provide <br />responses to the questions below. All other applicants, please skip these questions. (Responses <br />should not exceed 100 words per question) <br /> <br />1. Please describe the impact the proposed programs will have on the target population. Please include <br />specific quantitative and qualitative data in your response. <br /> <br />The impact of this program creates a safety net for disenfranchised populations. Providing <br />direct services and resources to over 100 families help to sustain households. $3,000 was <br />provided in gift cards for twenty Carrboro families for four months. Food collection and <br />distribution was provided to 80 families living in Elliott Woods and Chase Park apa rtments. <br />500 turkeys were given away all over Orange County during the holidays. Facilitated virtual <br />Listening Sessions to monitor the experience low -income housing residents had with the <br />Emergency Housing Assistance. Utility bill payment assistance to 67 households preventing <br />them from facing debt and disruption of utilities . <br /> <br />2. What methods/tools will your organization use to evaluate the proposed program’s effectiveness? <br />Please include specific examples, such as a logic model. <br />EI will use several methods to evaluate the program’s effectiveness. Electronic surveys were <br />used for the Listening Sessions. The utility assistance tracking is done with applications and <br />one-on-one budgeting sessions. The food and gift certificate give -away programs require <br />information supplied through an in-take process usually accessed via text and emails. <br />Demographics are collected from the various methods to verify the populations being served. <br />Collected data is analyzed at the staff and board level. <br /> <br />3. Please briefly describe how your proposed programs aligns with evidence-based approaches to <br />addressing human service need(s). <br />Years of data verify housing stability is the foundation of stable lives. Programs, like <br />EMPOWERment’s Affordable Housing Program align with that data. By purchasing rental <br />units that will be rented to families earning less than 60 AMI, helps build that foundation. <br />Affordable rental for families formerly sleeping on the couch of family of friends restores <br />respect, dignity and creates a social safety net through housing. <br /> <br />4. Please describe one to three key partnerships/collaborations that add the most value to the success of <br />the proposed programs. <br /> <br />Family Success Alliance - major factor in reaching the Latinx communities.
