Program Services Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2023. • PORCH CHC will ensure that families receive the food monthly (with the use of effective communication in first languages of participants, distribution at central drive-through site in the community, deliveries when needed, maintenance of updated contact information, and ongoing relationship building) • PORCH CHC will raise enough funds (at least $35 per month per family) to provide substantial quantities of food to targeted number of individuals enrolled in program through grant writing and fundraising efforts • PORCH CHC will provide food that is desirable based on feedback from program participants, as well as provide recipes and nutrition information, to encourage consumption of healthy, fresh food by program participants Anticipated Outcomes • The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within that measure’s description or for an earlier performance measure.
