Please briefly describe how your proposed programs aligns with evidence-based <br />approaches to addressing human service need(s). <br /> <br />The core of our service is the delivery of consistent, nutritious meals and friendly <br />check-ins. Access to good nutrition is critical as older adults are often at risk of <br />malnutrition given the biological, social, economic and functional challenges that often <br />accompany aging and limit a senior’s ability to acquire, prepare and consume nutritious <br />foods. Research suggests that receiving MOW services reduces food insecurity <br />rates by up to 28%. Moreover, MOW recipients report feeling less isolated, less <br />anxious about aging in place and more secure in their homes reducing the negative <br />health consequences of isolation and associated healthcare costs. <br /> <br />4. Please describe one to three key partnerships/collaborations that add the most value to <br />the success of the proposed programs. <br /> <br />• Meals on Wheels America: Resource for studies, research, data, program <br />development, and implementation strategies. Collective knowledge and practice <br />from MOW’s across the US. <br />• Chapel Hill, Carrboro and Orange County: provide consistent funding through <br />human service grants. <br />• UNC student groups: in the past UNC groups have designed and tested our <br />recipient survey, investigated differing needs between suburban and rural older <br />adults, developed pilot program to test delivery of medically tailored meals and next <br />semester will be investigating and analyzing opportunities for MOWOCNC to <br />improve the mental health (specifically social isolation and loneliness) of MOW <br />recipients. <br /> <br />5. If you are not awarded a Community Impact Award, what would your agency’s funding <br />request be? <br />As much funding as possible to enable MOWOCNC to sustain increased meal delivery <br />service in Chapel Hill, not only in actual meals delivered but more than a meal services <br />combatting social isolation and loneliness, emergency food and supply needs, and <br />resource connector. <br /> <br /> <br />DocuSign Envelope ID: 3E214CCE-9F32-46C0-9591-DFA25EDE6B6C
