Continue work on Behavioral Health and also <br /> Move to Alliance <br /> • Completed transition to Alliance Created Behavioral Health Crisis Diversion <br /> Health as the County's Managed Care Facility planning teams to issue Requests <br /> Organization for Qualification for planning and design <br /> and continued outreach and information to <br /> local municipalities and interested partners <br /> and agencies continues. <br /> • Worked with Alliance Health to Used MOE funds to implement contract <br /> transfer services for vulnerable foster with mental health provider to serve <br /> youth and adults in guardianship uninsured children in child protection cases <br /> services. Identified areas of concern <br /> and service gaps. Assured <br /> Department of Social Services staff <br /> representation on numerous <br /> committees and work groups and <br /> established relationships with new <br /> behavior health liaisons. <br /> • Maintained five teams of Telephone Provided 18 programs providing <br /> Reassurance volunteers— providing educational information on topics ranging <br /> assistance with social isolation and from End of Life Choices to Mental <br /> wellness needs for—65 older adults. Wellness support and resources or social <br /> engagement programs. <br /> • Provided licensed clinical social work • Provided harm reduction resources to <br /> assistance to 10 persons through the justice-involved individuals at risk of opioid <br /> Aging Transitions program overdose <br /> • Worked closely with Alliance Health • Created the Lantern Project with grant <br /> to improve access to care funds to improve deflection, diversion, re- <br /> coordination and services for their entry and harm reduction education and <br /> Orange County members who are services to justice-involved individuals with <br /> justice-involved. Worked with Alliance substance use disorders, opioid use <br /> to create a Crisis Collaborative of disorders and mental health concerns <br /> behavioral health crisis service <br /> stakeholders that meets monthly. <br /> 10 <br />
