The state Medicaid agency or <br />Department of Health & Human Services may <br />transmit the data, depending on the individual <br />state’s functional responsibilities. <br /> <br />Note : The July 30, 2022 submission of Core <br />Child Services data will include calendar years <br />2018-2021 data and data collected through <br />March 31, 2022. <br /> <br /> Quarter 4: October – December 2022 <br />Operational Plan and timeline <br /> <br />Due : October 15, 2022, with <br />NCC Application <br /> <br />The Operational Plan with corresponding timeline is an <br />annual submission requirement. The Operational Plan <br />outlines how the Implementation Plan will be executed <br />by laying out the intended key activities for the <br />upcoming 12-month Budget Period, and a <br />corresponding timeline for meeting milestones (i.e., <br />convening the Partnership Council, sharing data among <br />Model Partners, conducting data collection and <br />reporting activities, monitoring outcomes, developing <br />timelines). <br /> <br />The Recipient shall submit the Year 4 Operational <br />Plan , corresponding timeline , and documentation <br />supporting the plan for the next Budget Period with the <br />non-competing continuation (NCC) application on <br />October 15, 2022. <br /> <br />See Section 11 (InCK Model Implementation Plan & <br />Operational Plan Documents) of this document for <br />additional details on this milestone. <br /> <br />Partnership Council Charter <br /> <br />Due : October 15, 2022, with <br />NCC Application , if <br />applicable <br /> <br /> <br />The Recipient shall resubmit its Partnership <br />Council Charter only if there are changes to the <br />charter or membership.
