16 | Page <br /> <br />Annual Progress Report <br />(APR) <br /> <br />Due : March 31, 2022 <br /> <br />The Recipient shall submit to both GrantSolutions (as a <br />Grant Note) and the InCK Portal (via the Milestone <br />Reporting page) an APR covering the progress <br />achieved during the previous Budget Period (2021). <br /> <br />Driver Diagram <br /> <br />Due: March 31, 2022, with the <br />APR, if applicable <br /> <br />The Recipient shall submit to the InCK Portal the <br />revised driver diagram as a separate attachment with <br />the APR covering Budget Period 2 (2021) only if <br />changes were made to the previously approved version. <br /> Quarter 2: April – June 2022 <br />Service Integration Level <br />(SIL) Data Elements <br />Workbook <br /> <br />Due : April 30, 2022 <br /> <br />Resubmission quarterly <br />thereafter, for the remainder <br />of the Budget Period <br /> <br /> <br />The Recipient shall provide identifiable, beneficiary- <br />level information documenting the results of each <br />beneficiary’s screening within a SIL Data Elements <br />Workbook on a quarterly basis, beginning 30 Days <br />following the last day of Quarter 1 (April 30, 2022). <br />The workbook, completed for each beneficiary <br />screened, includes an overall tab with the assigned SIL <br />and 12 individual tabs for each assessment area. <br />Recipients must populate these tabs with beneficiary- <br />level data, treating each tab as a separate file. When <br />completed, the workbook must be submitted by award <br />recipients through the InCK Reusable Framework. <br /> <br /> Quarter 3: July – September 2022 <br />Reporting for Measures in the <br />Food and Housing Domains <br /> <br />Due : July 30, 2022 <br /> <br /> <br />The Recipient shall submit food and housing measure <br />sets 1 annually through the InCK Reusable Framework, <br />beginning July 30, 2022. Thereafter, annual submission <br />shall occur 30 Days following the last day of Quarter 2 <br />in every Budget Period. <br />• This first submission will capture data <br />from January 1, 2022 - March 31, 2022 <br />(reported by July 30, 2022). <br />• The second submission (in Budget Period <br />4) will capture data from April 1, 2022 to <br />December 31, 2022 (reported by July 30, <br />2023). <br /> <br />1 For MY3 (2022) only, CMS is relaxing enforcement related to the requi rement to report data on food and housing <br />measures for at least 80 percent of the attributed population. CMS does not plan to take corrective action based <br />solely on the Recipient missing this threshold in MY3.
