Recipients shall submit the list through the RF <br />unless CMS designates an alternative mechanism prior <br />to the first submission. <br /> <br />Final Needs Assessment and <br />Stratification and Service <br />Integration SOPs <br /> <br />Due : March 30, 2022 <br /> <br />Res ubmission with NCC <br />Application for Budget Period 4 <br /> <br /> <br />The Recipient shall submit as a Grant Note via <br />GrantSolutions both finalized SOPs to CMS for review <br />and acceptance by March 30, 2022. Recipients’ final <br />SOPs must address all CMS feedback and be compliant <br />with model policy, as outlined in the SOP Review <br />Criteria provided to Recipients in MY2. <br />To obtain CMS approval of the SOPs, the Recipient <br />shall resubmit the latest CMS-accepted versions of both <br />SOPs as attachments to the Year 4 Operational Plan <br />submitted with the non-competing continuation (NCC) <br />application for Budget Period 4 (due October 15, <br />2022). <br /> <br />See Section 12 (Standard Operating Procedures and <br />Needs Assessment Tool(s)) of this document for <br />additional details . <br /> <br />Selected InCK Needs <br />Assessment Tool(s) <br /> <br />Due: March 30, 2022 <br /> <br />Resubmission with NCC <br />Application for Budget Period 4 <br /> <br /> <br /> <br />The Recipient shall confirm its selection of needs <br />assessment tool(s) and submit the tool(s) as a Grant <br />Note via GrantSolutions to CMS for review and <br />acceptance by March 30, 2022.
