Outside Agencies/Human Services <br /> <br />Program information P a g e 37 o f 55 <br />garden as an educational and wellness activity and <br />engages community experts for workshops and <br />trainings related to wellness. <br />Staff work closely with members who have individual <br />goals specifically related to healthy eating and <br />increased wellness to ensure they have the supports <br />needed. <br /> <br />Club Nova will: <br />• Maintain records of the number of meals served daily. <br />• Maintain daily records of individuals eating at the <br />Clubhouse. <br />• Survey its membership yearly to evaluate the <br />achieved health and food security benefits of our <br />healthy, affordable meal program and nutrition <br />education. <br />• Staff will closely monitor specific individuals’ wellness <br />goals through progress notes. <br />Actual Results <br />(Outcome) <br />Ending FY19-20 <br />Club Nova partnered with Farmer Foodshare to secure <br />437.5lbs of fresh produce by mid-March when Farmer <br />Foodshare suspended their operations. <br />100% of the 105 Club Nova members who physically <br />attended Club Nova had access to an average of 13 <br />healthy, well-balanced meals each week (5 breakfasts, <br />6 lunches and 2 dinners); When the physical facility <br />closed in March, Club Nova began offering 7 meals <br />prepared and delivered weekly to members who <br />requested meals. Prior to the pandemic Club Nova <br />served roughly 7,000 meals.
