Outside Agencies/Human Services <br /> <br />Program information P a g e 31 o f 55 <br /> <br />Program Goal # 4 <br /> <br />Club Nova members will maximize their recovery by <br />participating in Club Nova’s comprehensive, integrated, <br />effective, community-based programming that <br />addresses the health, safety, social, and economic <br />risks of serious mental illness. We accomplish this goal <br />through providing the following: <br />•Assistance accessing and navigating quality <br />healthcare <br />•Supports to secure/maintain safe, affordable, dignified <br />housing <br />•Crisis prevention and intervention <br />•Assistance securing and maintaining benefits <br />•Nutritious, affordable meals <br />•Outreach calls and visits to combat isolation and <br />loneliness <br />•Transportation to access Club Nova (already included <br />in Orange County request) <br />•An Employment/Education Program: Addresses high <br />unemployment rates and interrupted educations <br />(Employment is covered in Orange County request) <br />•The Work-Ordered Day: Provides a sense of <br />community, common purpose, and opportunities to <br />contribute; building confidence, skills, and self-worth <br />•Social Program: Addresses isolation and social <br />connectedness <br />•Technology devices for members during the pandemic <br />including cellphones to be used for zoom meetings <br />offered by the clubhouse and tablets for members to <br />use to engage in Zoom activities offered by the <br />clubhouse during the pandemic <br />•Cell phone internet data for members to connect to the <br />work ordered day offered by the clubhouse and to <br />increase social interactions between members and <br />staff during the pandemic <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />DocuSign Envelope ID: 11DA72B8-CC83-439E-A25C-F64DE607C614
