Outside Agencies/Human Services <br /> <br />Program information P a g e 28 o f 55 <br />Program Goal # 2 <br />The goal is for Club Nova to offer the full spectrum of <br />services to ACTT members (typically those members <br />that experience the most disabling effects of mental <br />illness) regardless of our ability to receive payment for <br />these services, due to ACTT/PSR exclusion, which <br />amounts to roughly $120,000 annually. The ability to <br />participate in Club Nova maximizes recovery. <br /> <br />Club Nova actively collaborates with the ACTT <br />providers to coordinate care and ensure the health and <br />safety support of these members. <br />Performance Measure <br />(How will you accomplish <br />your goal?) <br />16 current members who receive Assertive Community <br />Treatment Team (ACTT) non-billable services (typically <br />those members who experience the most disabling <br />effects of mental illness) will continue to receive full <br />access to the supports and opportunities provided by <br />Club Nova. <br />Actual Results <br />(Outcome) <br />Ending FY19-20 <br />Club Nova: <br /> Monitored our provision of services to ACTT <br />members through calculating monthly <br />participation and attendance hours. <br /> Wrote monthly service notes on each member <br />receiving ACTT services. <br /> 100% or 16 Club Nova members who received <br />ACTT services maintained their Club Nova <br />membership, and received supports and <br />services provided through Club Nova. <br /> Due to COVID-19 pandemic and the closing of <br />the Clubhouse on March 14, 2020 the number of <br />ACTT members participating in the Zoom <br />activities offered by the clubhouse is 44% or 7 <br />ACTT members. <br />Projected Results <br />(Outcome) <br />Ending FY2021 <br />100% or 117 Club Nova members who receive ACTT <br />services will maintain their Club Nova membership and <br />receive supports and services provided through Club <br />Nova. <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />DocuSign Envelope ID: 11DA72B8-CC83-439E-A25C-F64DE607C614
