DocuSign Envelope ID:6A1278D6-4287-4002-98A9-C69C8C1D313E <br /> Indoor Environmental Air Quality Assessment <br /> • The Contractor acknowledges that he/she has acquainted him or herself with all conditions that <br /> may affect the work as would be evident from a thorough investigation of the job site and these <br /> specifications covering the work. <br /> • Should any error or inconsistency appear in the Specifications,the Contractor,before proceeding <br /> with the work,must make mention of the same to the project coordinator for proper adjustment, <br /> and in no case proceed with work in uncertainty. <br /> Remediation Contractor Responsibilities <br /> • The remediation contractor is responsible for the following: <br /> o Damage to property during the mold abatement procedures. <br /> o Failure to achieve a passing post-remediation verification(PRV)assessment. <br /> o Compliance with all federal, state and local codes as applicable to remediation activities <br /> to include holding permits as required by government entities. <br /> o Security of abatement equipment. <br /> o Communicating with the property owner/manager and obtaining permission for unusual <br /> requirements, such as but not limited to air discharge procedures, electrical requirements, <br /> temporary water, operations and/or storage areas, and requirements for access to the <br /> property. <br /> o Communicating with the property owner regarding concerns about lead-based paint and <br /> asbestos hazards(additional testing for lead/asbestos hazards may be required). <br /> o Staffing the project with full-sized crews. <br /> o Keeping an on-site folder containing the remediation procedures,route to a local hospital, <br /> project contacts with telephone numbers,Daily Field Activity Logs, and any other <br /> documents required for successful completion of the project. <br /> o Fire extinguishers to be set inside the containment area. <br /> o Signage. <br /> o The on-site supervisor identified by the Contractor will continue with the project until <br /> completion. <br /> o The Contractor must agree to the time schedule indicated by the property owner/manager <br /> that the property is available for abatement. <br /> o The Contractor must agree to the project documentation requirements identified in the <br /> Qualifications section of this Report. In addition,the Contractor should provide ESG <br /> with the qualifications of key personnel and any subcontractors,an example of daily log <br /> forms, a list of chemicals to be used with their MSDS sheets,confirmation of how waste <br /> will be packaged, decontaminated and disposed of,the Respiratory Protection and Health <br /> and Safety Plans, and proof of E&O Insurance that covers mold abatement activities. <br /> Health& Safety <br /> • The remediation contractor is responsible for the health and safety of remediation personnel and <br /> occupants of the building and is responsible for the following: <br /> • Providing appropriate personal protective equipment(PPE)to their employees. <br /> • Providing fire extinguishers inside the work area. <br /> • Providing an orientation for workers upon arrival to the site to discuss safety, security, egress, <br /> waste disposal, discretionary requirements, and decontamination procedures. <br /> • Posting of warning signs at entrances to the work area that only authorized personnel are to enter. <br /> Environmental Solutions Group • 7 Maple Leaf Drive,Ste 104, Greensboro, NC 27410 • 336-373-1538 30 <br /> www.Go-ESG.com <br />
