Pascal Director, ext. 2007 <br /> Agreement <br /> TELEPHONE NUMBERS: <br /> Hillsborough 7324181 <br /> Chapel Hill 9684501 <br /> Durham 688-7331 <br /> Mebane 336-227-2031 <br /> PURPOSE: To seek authorization to provide on-sde psychosocial assessments and service <br /> coordination for the residents of First Baptist and Manley Estates senior housing complex in <br /> Chapel Hill with the goal of facilitating successful aging-in-place and the postponement of long- <br /> term care institutionalization. <br /> BACKGROUND: HUD housing complexes for seniors are designed for independent seniors; <br /> therefore, no services such as meals, housekeeping, or personal care services are provided on- <br /> site by the management. As seniors ags-in-place,their independence may be compromised by <br /> physical andlor cognitive declines. Having a social worker on-site at the complex to provide <br /> regular psychosocial assessments, collaboration on problem solving, and assistance with the <br /> coordination of community resources should extend the time that seniors can live safely and <br /> comfortably in this affordable setting. <br /> The Master Aging Plan induced an objective (Goal I I-B/s-2)to assist the new HUD 202 <br /> Housing for the Elderly Project in the provision of support services and study contracting <br /> provision of service coordination. HUD makes available to its properties a monthly stipend per <br /> resident for the purchase of such service. Often,the service coordination Is contracted to an <br /> outside consultant. If the consultant encounters a situation that needs further assistance, they <br /> make a referrsl to the Department on Aging or other local Gore provider. The Department on <br /> Aging proposes to provide the service coordination directly for this new facility, utilizing social <br /> work interns under me direction of the master'a prepared social work staff. Thefacildy is <br /> scheduled for dedication on January 27, 2002. These resident assessment and re-assessment <br /> services will result In better access to community resources and to more intensive case <br /> management services, should the need arise. <br />
