3 <br />NC ESG FY 2021-22 Scope of Work <br />requests have been both incurred and paid. The Subrecipient is also required to use the forms provided by the NC ESG Office. <br />Any deviations from the minimum documentation standards or the NC ESG provided forms, must be approved by the NC ESG <br />Office prior to use. <br /> <br />Ineligible Activities <br />If the Subrecipient provides emergency response and/or housing stability activities to ineligible participants using NC ESG <br />funds or NC ESG matching funds, the Subrecipient will reimburse the State for those funds from a non-ESG funding <br />source. <br /> <br />NOTE <br />•A submission of ineligible expenses renders the entire requisition ineligible <br />Performance Monitoring and Assurance Plan <br />Monitoring is an integral management control technique and a Government Accountability Office (GAO) standard. It is the <br />activity that the NC ESG Office establishes and operates to assess the quality of performance over time and promptly resolve <br />the findings of audits and other reviews. Monitoring provides information about program participants that is critical for making <br />informed judgments about program effectiveness and management efficiency. It also helps in identifying instances of fraud, <br />waste and abuse. It is the principal means by which the NC ESG Office: <br /> <br />A.ensures that NC ESG funded programs and technical areas are carried out efficiently, effectively, and in compliance <br />with applicable laws and regulations; <br /> <br />B.assists program participants in improving their performance, developing or increasing capacity, and augmenting <br />their management and technical skills. <br /> <br />Subrecipients are expected to make available all participant level, financial, and program records for periodic review. Significant <br />deficiencies in file content or quality will result in required Plans of Corrective Action, with possible loss of allocated funds upon <br />discovery of continuing deficiencies. While it is the practice of the NC ESG office to request randomly selected files prior to a <br />monitoring visit, files must be available on demand and produced within a reasonable amount of time after on-site arrival. <br /> <br /> NC ESG Office will regularly monitor HMIS/comparable database reporting to verify consistent data entry for applicable <br />program activities. NC ESG Office reserves the right to request additional reports as necessary to support the NC ESG <br />program. All Subrecipients are required to: <br /> <br />•Provide aggregate data (CAPER) to the NC ESG Office for reporting purposes. <br />•Participate in the annual Statewide Point-in-Time (PIT) / Housing Inventory (HIC) count. <br /> <br />PERFORMANCE STANDARDS <br />FEDERAL / STATE REQUIREMENTS <br /> <br />Accessibility of Spaces, Services and Activities <br />Subrecipient shall operate each existing program or activity receiving federal financial assistance so that the program or activity, <br />when viewed in its entirety, is equally accessible to and usable by individuals with disabilities as by persons without disabilities. <br />Affirmatively Furthering Fair Housing <br />Under section 808(e) (5) of the Fair Housing Act, HUD has a statutory duty to affirmatively further fair housing. HUD requires the <br />same of its funding recipients. Subrecipients will have a duty to affirmatively further fair housing opportunities for classes <br />protected under the Fair Housing Act. Protected classes include race, color, national origin, religion, sex, disability, and familial <br />status. Examples of affirmatively furthering fair housing include: (1) marketing the program to all eligible persons, including <br />persons with disabilities and persons with limited English proficiency; (2) making buildings and communications that facilitate <br />applications and service delivery accessible to persons with disabilities (see, for example, HUD’s rule on effective <br />communications at 24 CFR 8.6); (3) providing fair housing counseling services or referrals to fair housing agencies; (4) informing <br />participants of how to file a housing discrimination complaint, including providing the toll-free number for the Housing <br />Discrimination Hotline: 1-800- 669-9777; and (5) recruiting landlords and service providers in areas that expand housing choice <br />to program participants. <br /> <br />CoC and Con Plan Participation <br />•Subrecipients are expected to attend no less than 75% of their CoC’s meetings. <br />•Subrecipients must participate in the CoC’s Point-in-Time and Housing Inventory Count <br />•Subrecipients must participate in the NC Consolidated Plan (2020 – 2025) <br />Contract Number 00043639 / Page 11 of 28 <br />DocuSign Envelope ID: F449611A-DCEC-4513-BEA0-00A72FB9421A
